IT SERVICE CHANGE FORM - The form which must be completed by supervisor and submitted to HR to add or remove IT Services for employees when hired, changing job roles, or leaving employment with SWCC.
--All employees of SWCC must obtain a security certificate by completing training to receive/maintain access to SWCC networked information including computers, printers, email, networked applications, AIS and SIS information.
--Employees will receive a notice for Certification Renewal each year on their individual certification date. Note: MOAT is accessible using Internet Explorer 5.5 or higher, Safari and Firefox, with pop-ups ENABLED for that site. See also www.sw.edu/moat for more information
